Title: Clinical Manager Department: Home Health & Hospice Requisition #: 227809 Hours: 40 Shift: Monday through Friday 8:00am to 4:30pm, and including weekend andevening on call rotation with supervisors. Union:Non-Union Posting Date:05/18/2022 Status: Exempt/Salaried Location:HealthAlliance, Leominster CampusHealthAlliance Home Health and Hospice has a 5 Star Patient Survey Rating Center for Medicare and Medicaid Services (CMS) Care Compare rating April 2022Position Summary: The Clinical Manager is responsible for the oversight of a clinical interdisciplinary team and leading the team in the provision of high quality, effective, and efficient clinical services. The Clinical Manager supervises, supports, and educates staff at the point of service/in the field. Maintains knowledge of current laws and regulations of federal, state, and regulatory bodies relative to home health and hospice. Works within those constraints to maintain actions within the agencys and corporations compliance programs.Major Responsibilities:

  • Oversees the management of the interdisciplinary home health team
  • Assures adequate and appropriate assignment of care, clinical integrity of practice, and education/support of team members is in place
  • Builds and evaluates team members including the employment, counseling, disciplinary action including termination (recruitment and retention efforts/activities).
  • Provides formal evaluation clinical competency in area of practice on skills assessment, direct observation and performance appraisals
  • Assures compliance with timely and accurate completion and submission/synching of all documentation
  • Collaborates with PI Team Leader and Director of Clinical Operations and staff regarding OASIS integrity, individual abilities and deficits in documentation and corrects
  • Assures compliance with the agency productivity expectations and clinical practices according to State and Federal regulations.
  • Encourages and fosters: Positive attitudes, and high morale toward establishing and maintaining a culture of patient focused service. Critical thinking the multidisciplinary team members and amongst discipline-specific clinicians. Understanding individual care planning and the agencys identified annual clinical and financial goals. Development and adherence to individual patient Plans of Care
  • Provides oversight and mentoring in the field related to the accurate capture of the clinical and functional impairment of each patient as documented in the OASIS assessment.
  • Provides direct management of : Interdisciplinary team of RNs, LPNs, PTs, PTAs, OTs, COTAs, STs, MSWs, and HHAs, Team Associates (Schedulers)

  • Takes appropriate action when departmental policy or professional standards are violated
  • Communicates with the Director of Clinical Operations, advising of issues of concern, offering suggestions for improvement
  • Participates in Performance Improvement activities of Agency
  • Utilizes available management data to monitor operations efficiency and guide team members in high quality, efficient, and effective care delivery
  • Manages all home health staff time off requests to assure safe and adequate staffing 7 days a week.
  • Meets established productivity standards.
  • Facilitates and promotes the sharing of knowledge and content throughout departments.
  • Takes responsibility for ensuring that all work outcomes satisfy the UMass Memorial Health System True North.
  • The individual must support the mission, vision, and goals of HealthAlliance-Clinton Hospital and serve as a role model for CARES values.

  • Attends staff meetings and in-service programs as required or directed. Keeps current with hospital and unit changes by reading communication boards and/or books, bulletin boards, posted notices and reads and responds to e mails on a regular basis.
  • Practices cost containment and fiscal responsibility through the efficient use of supplies, equipment, time, etc.
  • Complies with established departmental policies, procedures and objectives.
  • Attends variety of meetings, conferences, seminars as required or directed.
  • Demonstrates use of Quality Improvement in daily operations.
  • Complies with all health and safety regulations and requirements.
  • Respects diverse views and approaches, and contributes in maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
  • Performs other similar and related duties as required or directed.

    Experience in Home Health preferredExperience in HospiceLICENSES, CERTIFICATIONS ETC:Valid MA license in nursing, physical therapy or speech/language pathologyPATIENT POPULATION SPECIFIC COMPETENCIES:
    Possesses and applies the skills and knowledge necessary to provide care to patients and customers throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.Department-specific competencies, including age-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
