Salary: USD $18.75/Hr.Description and Responsibilities:Open
Sky's Autism Spectrum Disorder program serves adults 18 and older
who are aging out of school based programs, that benefit from
continued supports to assist in achieving their life goals and
dreams. Services provided to individuals include assessment, case
management, individual supports, employment/education supports, and
in home services. All members of the team are highly trained with
extensive experience in clinical models and working with
individuals with Autism Spectrum Disorder. We are known for the
rigor in which we train, supervise, and support our staff to
deliver the best possible service and treatment.The ASD program is
located in Worcester, but serves individuals living in the
communities of Central, South Central and North Central
Massachusetts, so travel is required for this position. Travel is
eligible for mileage reimbursement!The Case Manager will provide
community based behavioral supports that are specific to the
individual served to allow them to live independently, in the least
restrictive setting in the community. Case Managers will teach, and
foster skills related to personal finance, health, shopping, use of
community resources, community safety, and other social and
adaptive skills to enable the participant to live within the
community. They also work to facilitate the exploration of, and
participation desired in one's community, promoting development of
leisure/recreational opportunities and coordination of day
supports, including supported employment and education.Other Key
Responsibilities:Refer individuals to employment supports that will
assist with employment readiness and maintain employment. This will
prepare participants to exercise control and responsibility over
services and supports they receive as they become more
independent.Enhance the quality of life by using Positive
Behavioral Supports.Engage with the individual's team in a
collaborative manner inclusive of behavior specialists, family
members, and other providers of support
services.Qualifications:High School Diploma or equivalent.
Alternative education/experience in lieu of formal education will
be considered.Experience working with individuals with Special
Education, Autism Spectrum Disorders (ASD), behavioral needs, is
preferred.Valid Driver's License, acceptable driving record, and
