Job Description:
Full Time Direct Hire RN's Wanted. Fitchburg MA. Must have MA RN
License. 40 hours a week, 4 k sign on Bonus, Salary
$60,000-$75,000.These are 100% outpatient .. no nights /weekendsIn
collaboration with the provider (PCP) the RN Care Coordinator helps
to identify, assess and coordinate the care for complex care
patients. These patients are defined as those at high risk for
health deterioration, sentinel events and /or poor outcomes. The
medical facility is out patient and 30-40 beds.
