Salary: USD $37.00/Hr.Description and Responsibilities:The
Assertive Community Treatment (ACT) Team is a multidisciplinary
team that provides an alternative to traditional outpatient
treatment to any person who needs it within the community. The ACT
team provides 24/7 care and continuity of care (open-ended,
lifelong) by mimicking hospital-level wraparound support to help
individuals maintain in the community rather than bouncing back and
forth from the hospital. We bring the support to our individuals by
meeting individuals where they're at, in settings and spaces where
they feel safe and comfortable. Our unique contract with the
Columbia Center for Practice Innovations provides the ACT program
robust, job-specific training to each ACT team member.The ACT
Registered Nurse is a member of a multidisciplinary team and aims
to understand, embrace, and operate within the ACT model. The
registered nurse is responsible for conducting psychiatric
assessments, assessing physical health needs, making appropriate
referrals to community physicians, providing management and
administration of medication in conjunction with the psychiatrist,
providing a range of treatment, rehabilitation, and support
services, and sharing shift-management responsibility with other
staff. The registered nurse collaborates with the team, the
individuals enrolled in services, and their collaterals to develop
and revise comprehensive assessments and individualized treatment
plans. They will exhibit Open Sky's commitment to racial equity,

RESPONSIBILITIES:In collaboration with the psychiatrist, develop,
revise, maintain, and supervise team psychopharmacologic and
medical treatment and medication policies and procedures including
transcribing, administering, evaluating, and recording psychotropic
medications prescribed by the team psychiatristEvaluate and
document psychotropic medication effectiveness, complications, and
side effectsProvide case management and care coordination support
including making referrals and building and maintaining
relationships with health services providers in the community
(e.G., community health centers, detox facilities, inpatient
psychiatric and medical units, etc.)Provide clinical and
non-clinical support in the form of psychoeducation to individuals
and families, educational and vocational supportive counseling,
assisting with ADLs, supporting individuals to maintain safe and
stable housing, teaching life skills, and symptom
managementParticipate in the on-call
rotationQualifications:Licensed registered nurse (RN) in the state
of MassachusettsMinimum of 1 year working with adults with serious
and persistent mental illness and substance use
disorders.Experience with local resources, navigating health,
behavioral health, and/or substance abuse treatment
systems.Excellent communication skills, documentation, time
management, and organizational skills.Valid driver's license and
reliable transportation.Proficiency with Windows computer
environments, including office software such as Word and
