Salary: USD $37.00/Hr.Description and Responsibilities:The Assertive Community Treatment (ACT) Team is a multidisciplinary team that provides an alternative to traditional outpatient treatment to any person who needs it within the community. The ACT team provides 24/7 care and continuity of care (open-ended, lifelong) by mimicking hospital-level wraparound support to help individuals maintain in the community rather than bouncing back and forth from the hospital. We bring the support to our individuals by meeting individuals where they're at, in settings and spaces where they feel safe and comfortable. Our unique contract with the Columbia Center for Practice Innovations provides the ACT program robust, job-specific training to each ACT team member.The ACT Registered Nurse is a member of a multidisciplinary team and aims to understand, embrace, and operate within the ACT model. The registered nurse is responsible for conducting psychiatric assessments, assessing physical health needs, making appropriate referrals to community physicians, providing management and administration of medication in conjunction with the psychiatrist, providing a range of treatment, rehabilitation, and support services, and sharing shift-management responsibility with other staff. The registered nurse collaborates with the team, the individuals enrolled in services, and their collaterals to develop and revise comprehensive assessments and individualized treatment plans.
