Job Description LanceSoft is seeking a Nurse Practitioner Long Term Care (LTC) Geriatrics for a local contract job in Troy, New York. Job Description & Requirements - Specialty: Geriatrics - Discipline: Nurse Practitioner - Start Date: ASAP - Duration: 52 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Local Contract About the Role Are you passionate about serving the geriatric population? We are seeking a compassionate and skilled Nurse Practitioner (NP) to deliver high-quality, patient-centered primary care services to residents in assisted living and long-term care facilities. This is a meaningful opportunity for providers who are dedicated to improving the health and quality of life for older adults in a supportive, team-based environment. Key Responsibilities - Conduct comprehensive health assessments including physical exams, medication reconciliation, and review of medical histories. - Develop and implement individualized care plans in collaboration with interdisciplinary teams. - Manage chronic and acute health conditions with timely interventions and appropriate referrals. - Order and interpret diagnostic tests (labs, imaging, etc.) to guide clinical decisions. - Prescribe medications and treatments in accordance with state scope of practice and facility policy. - Initiate and participate in chronic care management programs. - Educate residents and families on health maintenance, disease prevention, and self-care. - Participate in quality improvement efforts and interdisciplinary care meetings. - Maintain clear, accurate documentation in the electronic medical record (EMR) system. Qualifications - Master’s Degree in Nursing from an accredited Nurse Practitioner program. - Active and unrestricted New York State NP license. - Valid DEA registration for prescribing. - Eligible for reimbursement through Medicare and Medicaid.
