Pay: $2,431 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 1/19/2026 About the Position Caledonia Home Health Care & Hospice is seeking a Physical Therapist up to 40 hours/week. The successful candidate will provide in-home therapy services to maximize the independence and safety of our home care and hospice patients. Must work well independently while also being a collaborative member of our multidisciplinary team. Requires VT licensure as a Physical Therapist, a valid driver s license and reliable vehicle. Experience in a home care setting is preferred.
