Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (40 hrs/wk) Case Manager in Leominster, MA.

The Certified Community Behavioral Health Center initiative within Community Healthlink is an agency-wide initiative to increase access to the full range of community behavioral health services and to improve the behavioral health and quality of life of the population of focus in Worcester and Leominster, MA through the certification of CHL as a CCBHC and the expansion of our community behavioral health clinic services that align with both CCBHC certification and the needs of our community.

The Case Manager provides all needed levels of supportive services and coordination of other essential services to individuals and families requesting services at Community Healthlink.

* Perform comprehensive case management intake and assessment of the supportive service needs. * Develop appropriate plans with clients for obtaining essential services. * Assist program manager with screening for behavioral health assessment intakes. * Complete initial behavioral health intake registration documentation including release forms. * Work collaboratively with the integrated team to coordinate the delivery of appropriate behavioral health and other support services, including coordinating meet and greets between client and clinical staff. * Make appropriate referrals to existing community agencies for additional supportive or housing services as needed within the community. * Provide ongoing case management services to assist clients with obtaining various entitlements and benefits (i.e., SSI, EAEDC, MassHealth and other health insurance needs, addiction services, and other community resources). * Provide transportation assistance to individuals as needed. Assist clients in accessing transportation through their health insurance and/or budgeting. * Perform outreach services in collaboration with the clinical team or other staff. * Co-facilitate groups and provide necessary preparation and group planning support. * Identify gaps in services and provide data for the purpose of client advocacy. * Participate in collaborative case conferences regarding service planning for clients. * Regularly attend staff meetings, clinical team meetings, team meetings, supervision and all other required meetings. * Prepare and maintain confidential client documentation. * Attend or participate in relevant trainings * Perform other related job duties * Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.


* Grade 5: Case Manager I - HSD/GED or HiSet required. * Grade 6: Requires a Bachelor's degree; preferably in the field of psychology or human services

* Knowledge of substance use disorder, mental health and related issues; familiarity with community resources and entitlements. * Strong analytical skills, including the ability to recognize problems, think creatively, and devise innovative solutions. * Experience with Outlook Suite (Outlook, Word, Excel, etc.). * Familiarity with client documentation, electronic medical records. * Bilingual in English & Spanish is a plus. * Must have a current valid US-issued drivers license and a registered, inspected and insured automobile for work related purposes including transporting clients. * Must be able to pass a CORI background check.
