quality home visiting services for infants and toddlers. Community
Healthlink (CHL) is looking for Physical Therapists-
Early Intervention who are interested in working
(5-24 hrs/wk). Flexible hours available - days, early evenings
and/or Saturdays on a fee-for-service basis in the North Worcester
Intervention (EI) team provides
strength-based, family-centered services for young children under
three years of age who have developmental delays or are at risk of
a developmental delay. Services are designed to meet the
needs of each infant or toddler, and the needs of their family. The
EI team provides services in the home/community setting with the
goal of enhancing the child’s physical, cognitive, communication,
social-emotional and adaptive skills.
- Provides direct
intervention services to children 0 - 3 and their
- Carries out
service coordination functions when appropriate as defined by the
Early Intervention standards.
- Assists in
transition planning prior to discharge from the
- Referrals to
outside agencies as needed.
- Maintains client
records and other administrative duties according to DPH and Lipton
Early Intervention policies.
- Attends staff
and/or region meetings as required.

- Performs related
duties as assigned.
- Adhere and
complete all CHL's protocols, policies, and procedures.
Intervention services are provided both in-person and/or by
regular, reliable, and predictable attendance.
- Ability to work
independently and contribute to the overall success of the

views and approaches, demonstrates Standards of Respect, and
contributes to creating and maintaining an environment of
professionalism, tolerance, civility and acceptance toward all
employees, patients and visitors.
- Masters Degree
MA PT license
Aid certification or obtain within 6 months of
of Public Health CEIS certification or eligible for

Minimum of 2
years experience working with young children and families
treats children with musculoskeletal, neurological and/or
cardiopulmonary deficits which may block normal
knowledge of typical infant/toddler development in perceptual/fine
motor, gross motor, cognition, language and communication,
self-care and personal/social areas.
ability to support positive child and family
functional intervention techniques appropriate for infants and
toddlers with atypical development
US-issued drivers license and ability to provide a registered and
insured automobile for work related purposes, including travel
between work sites
Must be able to
pass a CORI background check
