Job Description:

Aspire Living & Learning is looking for a Shared Living Coordinator. The Shared Living Coordinator is responsible for monitoring and maintaining shared living arrangements for developmentally disabled individuals. He/she will ensure adherence to all funding agencies and their relevant regulations, as well as all Aspire rules, policies and procedures. The Shared Living Coordinator is responsible for the overall health and safety of all individuals on his/her caseload and ensures that all individuals are afforded opportunities to experience community activities, develop relationships, make individual choices and decisions, learn new skills in all levels of living, and are treated with dignity and respect.Some Essential Functions of the job include, but are not limited to:

  • Monitors shared living arrangements and implements relevant supports as needed;
  • Ensures that the maintenance of all systems meet or exceed Aspire standards and regulatory requirements of all funding agencies;
    • Makes certain that all providers he/she oversees keeps current and up-to-date with all trainings and certifications;
    • Mentors shared living providers as needed;
    • Works with Shared Living provider to coordinate routine and preventative medical services for individuals;
    • Oversees Medication Administration and related documentation;
    • Works with Clinical Director and/or assigned clinician to ensure all behavioral/clinical needs are being addressed;
    • Collects, tabulates and analyzes behavioral data used to develop and evaluate the effectiveness of behavior treatment plans and programs;
    • Provides financial oversight of individuals' funds;
    • Works with the Shared Living provider/AD of Adult Services: Shared Living to develop a program of inclusion activities that are consistent with the likes and abilities of each individual.
    • Ensures the growth, independence, autonomy and safety of each individual via ISP and informal life planning goals;
    • Maintains professional, positive, open dialogue and relationships with all internal and external contacts;
    • Attends, participates, prepares materials for and presents at ISP, HRC, PBS:
    • Targeted and Intensive committee's, other Aspire committees and clinical team meetings as required;
    • Responsible for on call duties as required;
    • Performs special projects and other tasks as required.KNOWLEDGE, SKILLS, ABILITIESMust demonstrate strong commitment to the Human Services field. Must have excellent interpersonal skills, verbal and written communication skills.CREDENTIALS - EXPERIENCE
      Bachelor's Degree or equivalent related experience in discipline related to the delivery of supports to individuals with developmental disabilities. Must have experience with shared living model.Must have MAP certification or have the ability to become MAP certified within timeframe specified by Aspire.
