Job Description:
Salary: USD $26.92/Hr.Description and Responsibilities:The South
County Integrated Care Team provides support and
clinical/rehabilitative services to individuals with mental illness
or dual diagnosis, who are living in South County communities. Our
services focus on helping people to transition to a more
independent environment while maximizing their natural supports by
building skills and achieving personal goals. The integrated team
includes a variety of clinical and non-clinical staff including
peer supports, recovery coaches, family partners, housing
coordinators, team leaders, outreach counselors, nurses,
clinicians, substance use counselors and licensed clinical Program
Director and Assistant Program Director. The team supports people
living in group environments including Litchfield, Bridge House,
and Transitions as well as people living in their own apartments or
homes.Masters Level Clinicals support individuals with mental
health challenges in Residential, Supported Housing and Community
settings. They will also complete all screening and enrollment of
persons served referred to ACCS in compliance with DMH regulations,
including initial and ongoing assessments for risk and protective
factors using agency risk assessment protocols and tools. In
addition, the Master's Level Clinician will develop and implement
Comprehensive Assessments and Individualized Treatment Plans using
Person Centered Planning, conduct individual sessions, facilitate
support groups and treatment planning meetings with individuals
served as well as provide clinical consultation and training to
staff on the integrated team.Other Key Responsibilities:Build
relationships with individuals served and develop shared
understanding of Person's goals and the steps necessary to achieve
goals. Along with this, all clinicians will demonstrate respect for
all Persons regardless of ability status, culture, sexual
orientation, gender identity, religion, or ethnicity.Collaborate
with applicable Care Coordination Entities, Emergency Service
Providers, outpatient clinics, and other providers/entities to
share information and ensure that the needs of the person are fully
met.Develop productive affiliations and partnerships with other
community-based providers to enhance the agency's service delivery

field, requiredMust be License Eligible or be a Licensed
Practitioner of the Healing Arts (LICSW, LCSW, LMHC. LRC, LADAC I,
physician, RN OTR/L or Psychologist).Salary will match level of
licensure/education obtained.Minimum of two years of professional
experience working.Valid Driver's License and acceptable driving
