Position Summary: This position is responsible for all weekend staff, referral intakes and coordination of patient care under direct report and supervision of the assigned weekend Clinical Manager/Administrator on call. Major Responsibilities: * Prepares patient schedules for the weekends and/or holidays. * Obtains and processes all physician orders during the weekends for follow up by the assigned weekend Clinical Manager/Administrator on call. * Performs intake responsibilities according to departmental procedures and policies. * Refers unusual referral requests to the Clinical Manager. * Provides effective direction and guidance to staff to achieve effective teamwork and motivation. * Interacts with external providers and accepts/enters referrals. Ensures that all pertinent clinical information is documented in the electronic medical record by all staff. * Ensures high patient satisfaction by minimizing the number of clinicians scheduled to visit patients. * Completes the weekend report for the transition to the weekday teams and managers. * Ensures case coordination and provides indirect nursing care to patients and families if needed from Saturday to Sunday. * Collects comprehensive data pertinent to the patient's health or the situation. * Analyzes assessment data to determine the diagnoses or issues. * Employs strategies to promote health, comfort, and a safe environment. * Systematically enhances the quality and effectiveness of his/her nursing practice. * Attains knowledge and competency that reflects current nursing practice. * Evaluates one's own nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules, and regulations. * Interacts with peers and colleagues and contributes to their professional development. * Collaborates with the patient, family, and others in the conduct of nursing practice. * Integrates ethical principles into all areas of practice. * Integrates best practices and/or research findings into practice. * Considers factors related to safety, effectiveness, and impact on practice in the planning and delivery of nursing services Standard Staffing Level Responsibilities: * Meets established productivity standards. * Facilitates and promotes the sharing of knowledge and content throughout departments. * Takes responsibility for ensuring that all work outcomes satisfy the UMass Memorial Health System True North. * The individual must support the mission, vision, and goals of HealthAlliance-Clinton Hospital and serve as a role model for CARES values. * Adheres to change control processes. * Participates in cross training to optimize department resources. * Demonstrates excellent attendance and actively participates in a variety of meetings and training sessions as required. * Demonstrates a friendly, responsive, service-minded attitude to all internal and external customers. * Communicates ideas effectively. Keeps current with hospital and unit changes by reading communication boards and/or books, bulletin boards, posted notices and reads and responds to e mails on a regular basis. * Practices cost containment and fiscal responsibility through the efficient use of supplies, equipment, time, etc. * Complies with established departmental policies, procedures and objectives. * Attends variety of meetings, conferences, seminars as required or directed. * Demonstrates use of Quality Improvement in daily operations. * Complies with all health and safety regulations and requirements. * Respects diverse views and approaches, demonstrates Standard of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors. * Maintains, regular, reliable, and predictable attendance. * Performs other similar and related duties as required or directed. All responsibilities are essential job functions. Position Qualifications: License/Certification/Education: Required: * High School Diploma * Completion of an accredited school of practical nursing program * Current and valid licensure as a Licensed Practical Nurse in the Commonwealth of Massachusetts Experience/Skills: Required: * Minimum two (2) years of health care experience * Valid and unrestricted driver's license recognized by the Commonwealth of Massachusetts and access to a reliable, insured vehicle * Excellent oral and written communication skills * Ability to speak, read and write fluently in English * Ability to act and communicate professionally at all times * Ability to effectively present information and respond to questions from patients, caregivers, managers, staff and external providers * Ability to lead, direct and motivate peers and subordinates * In depth knowledge of department productivity standards, policies and procedures * Excellent organizational skills and attention to detail * In depth knowledge of department productivity standards, policies and procedures Preferred: * Minimum one (1) year of home care / hospice experience Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. Department-specific competencies and their measurements will be developed and maintained in the individual departments.
